As an Albany Eye Associates patient, your insurance claim filing will be provided by our on-site fully trained patient accounts staff. We are ready to assist you with any of your questions regarding your account or insurance coverage. We would be happy to clarify any coverage questions that you may have.

Pre-registration via the telephone is provided to ease your check-in process on the day of your first appointment. Our patient registration form is available on this site, for you to print and fill out prior to your appointment, to ease the process as well.

Albany Eye Associates participates with most insurance plans. If your insurance carrier is not listed below, we suggest you contact them directly.
